To obtain your Indiana Choose Life License Plate:
- It is no longer necessary to obtain a Choose Life plate authorization form from this website.
- Effective January 2, 2008 you may go directly to the Indiana Bureau of Motor Vehicles (BMV) to
obtain your Indiana Choose Life plate. The BMV will collect their specialty plate fee, as well
as the $25 fee that goes to the Indiana Association of Pregnancy Centers.
- Funds raised by the sale of the Choose Life license plate go to support life-affirming
pregnancy resource centers in Indiana.
